ACP Day

Advance Care Planning Day is April 16^th. Advance Care Planning is thinking, talking about, and writing down your future health care wishes or instructions. It is your “voice” to let people know what kind of health care and treatment you would want in the future if you were unable to speak for yourself. On April 16^th, Hospice PEI, and Health PEI encourage you to have the conversation with family, loved ones, and your health care professional, as it is an essential conversation for everyone.
